OBRC Documentation Forms

The use of a special form for documenting unusual bird sightings is not necessary, but you might find it useful. You may fill out and submit the online documentation form below, or download a form you can print by selecting Microsoft Word format or PDF format which uses the free Adobe Acrobat Reader. Photographs and/or sketches are particularly useful in documenting bird sightings in addition to written descriptions. For tips on documenting your sighting, click here.
